1. A copy of your national statistical report.
2. District dues of $1 per member. (Not attendance like the national dues). This check should be made out to Grace Brethren Conference of So Ca/ Arizona
3. Your completed ballot for the election of officers (this was emailed to you).
4. Your personal Ministerium dues. $25 is for the District and $100 is for the National Ministerium. A combined check can be made out to So Ca/Arizona Ministerium and brought to Ministerium or mailed to Roy Polman at Cherry Valley.

March, 2012 John McIntosh filled the position of senior pastor at Grace Brethren in Simi Valley from 1983 through 2012. In March of 2012 John relinquished the Senior Pastor position to Jordan Bakker and assumed an associate pastor role. John is committed to God's Word and ministers by the motto, "the Bible, the whole Bible and nothing but the Bible."
